会費規約　Membership Fee Agreement

入会時に必要なもの

  • 入会金　

  • 当月の会費

  • スポーツ保険の申込料　年間2000円（毎年4月1日更新、3月31日解消）

休会、退会、プラン変更時は変更月の前月２０日迄にお申し付け下さい​

When Registering 

1, Filling out Registration form

2, Filling out waiver form

3, Registration fee

4, Monthly fee

5, Next month fee

6, Bank or Yu-cho account information for monthly withdraw and the stamp

7, Registering sports insurance　2000yen Annually(April 1st to March31st)

 

When suspending membership

1, Suspending notification by a written form in person (must notified by the 20th of two months prior)

2, Squaring of account

道場利用エチケット Dojo Etiquette

・入館時、退館時に入念な手洗いうがいを履行しましょう

 

・清潔な体、道着で稽古に臨みましょう

 

・手足の爪を短く清潔に切りそろえましょう

 

・利用した器具は汗などを拭きとりきれいにしてから、元の場所に必ずもどしましょう

 

・クラス終了後30分以内にマットから離れましょう

 

・When Entering and Leaving Dojo, please wash hands and gargle thoroughly

 

・Always keep your body and dogi clean

 

・Always cut finger nails and toe nails clean and short

 

・After using equipment, wipe sweat and clean before putting back to original spot

 

・Should leave the mats within 30 minutes after finishing your class
